Not sure if this has been mentioned on this thread or not. If you save your receipts of purchased apps, books or music, there will also be a Refund link within the email that will take you to the form. If you have deleted the email, or you delete these messages automatically when they come in, then definitely bookmark the link. I have gotten in the habbit of saving all the receipt emails for each item I have purchased from Apple. I have only had to get a refund once, but I was super glad I had the email with the refund link when that time came.
