I have also noticed it in other apps, like Facebook, in notifications, when you 
start swiping through your notifications, it will skip the first one like it is 
not even there.  So it is not a bug with a particular app, but a voice over bug 
in general.
We all just need to make sure that we report it to apple accessibility and hope 
it is corrected when ios 9 comes out.
Again, I did not have this happen in ios 7, but started with ios 8 back in 
September.
