If one just backs up to the cloud regularly, not backing up using iTunes, and needs to restore the phone at some future time, will they have to manually download each app again from the App Store, then restore from the last iCloud back up? Or, is iCloud smart enough to also get the App store to redownload all apps?

It does not back up apps, it will back up your data such as contacts. For a true backup use iTunes and backup the phone that way. That will backup all your apps and data.
