It seems that at least on my 5S I have to double tap the start button twice and the second time it says "Go" which I assume when visually it goes green. It then makes a sound when I put it down at the end point. As far as taking a measurement between 2 parallel surfaces, keep in mind that it says "Take a measurement between two parallel surfaces using the length of your iPhone" or something like that. I think the way this works is that you would put your phone down, put your finger wher it ends, then put the top of the phone against your finger, move your finger back to the end and repeat the process. The app knows the exact length of the phone so it probably just adds up how many times you move the phone. This I smy interpretation, I could of course be wrong and maybe somebody will find out for sure.
