I just experienced a bit of this when someone replied to one of my Facebook comments. When I double tapped on the initial post with the latest Facebook version on my iPhone 6, her reply was shown indented below mine but VoiceOver didn't read this indented comment. I had to double tap on the screen with the indented comment and a new screen opened where VoiceOver could read my comment and the indented reply. You might try that.
