I'm still getting the unpredictability primarily with opening apps. I've found that if I go into Settings, General and turn Siri off and back on again, she'll successfully open the app the next time I ask her. This is a pain to have to do and it doesn't seem to stick before she'll be back to saying "I don't see that app on your device" or some such thing. I'm not convinced it's an iOS version issue either but I sure wish I could figure out what causes this and come up with a workaround.
