I've been having a problem with Voice Over when using other audio apps such as 
Roger; in this case, it often locks the audio to the ear-piece on the phone, 
and won't use the speaker or the bluetooth headset, and restarting voice over 
won't solve the problem, I do have to reset the phone to get things operating 
normally again.

I did the triple click of home to shut it down and bring it back. So 
this doesn't require a complete phone restart on my end. So you can try 
that if you had been previously restarting your phone.
