Hi, the best way I have found to do this is by first making sure your typing is set to standard instead of touch. This means that when you tap on a letter, it behaves just like any other button or control on the iPhone. This means that you have to double-tap after first selecting the letter. Once you have it set this way, you can double-tap and hold on the letter you want, the letter A for example, then as you hold on the screen, drag it left or right and you will hear the various accented letters. Just release your finger when you get to the one you want and it will be entered.

On a side note, since I don't care for standard typing, I prefer the touch typing, what I tend to do is use DDragon dictation. I can set that to Spanish, and then I can speak whatever I want. I have to use Dragon because Siri doesn't currently understand Spanish. I think you have a 3GS, so Siri is not an issue anyway.
