Where exactly do you go for this? If you refer to the "Audio Destination" item which is present in the rotor if, for example, you turn on a bluetooth headset or speaker then changing this will simply toggle audio between your device and your bluetooth device, you cannot have Voiceover speak on the device and let's say music or an audio book on the headset or speaker.
