This is a really stupid question, but at least it's worth a shot… Have you 
rebooted your iPhone recently? Try rebooting both your computer and your 
iPhone. If that doesn't work, I would give the accessibility answer desk a call 
to see if they can troubleshoot the issue. They might be able to do something 
on their end.
