Sorry, but I completely disagree with the advice or theory here stated as "battery conditioning". According to many articles from authoritative sources (such as Pupular Mechanics), this isn't necessary with today's "smart" lithium ion batteries.

Batteries don't last forever, and if you don't condition them correctly, meaning, let them run completely flat, every month or 2, then doing a full charge, without breaking that charging process, it could give you the issues, that you are currently experiencing. If i were you, i would take it into an apple store, and let them run a full diagnostics on your handset.
