There can be situations where some app may have been corrupted in some way 
where it may use cellular or WiFi in the background and Resetting Network 
Settings can help in that it may force such an app or process to be terminated.
If a reboot or Reset Network Settings doesn't help doing a complete reset of 
the phone is the next step, best to have a backup, but first set up as a new 
phone and observe for a day or two if all is well, then restore the backup. If 
the problem comes back then whatever the problem is was part of the backup and 
then another reset is in order, this time the phone needs to be again set up as 
a new phone and your friend has to start from scratch, download apps, put back 
any content manually etc.
Maybe you recall how Mario was bitterly complaining that iOS 12 ruined the 
battery life on his iPhone SE and he mentioned some other issues also which I 
don't recall. Eventually he did a complete restore and set up as a new device 
and he reported that all was well after that. I know it's sucks and can be a 
lot of work depending on how many apps one has and how much stuff otherwise, 
but it is what it is.

Making sure to turn off background app refresh, screen time, WiFi Assist under 
Cellular, anything else that might be doing stuff behind the scenes.
I know you said he doesn't have any apps running, but he should turn off the 
background app refresh anyway and see how things go.

I'm going to assume this wasn't right after updating to iOS 12.0.1, because as 
others have noted, the battery drain can be large for the first day or two 
after an update.

I’m writing this on behalf of a friend who’s not on this list. He recently 
purchased an iPhone 8, and he is finding that his battery is not lasting well 
at all. For instance, overnight with the screen locked and nothing running in 
the background, he went from 100% to 43%. I seem to recall somebody saying that 
resetting network settings helps, although I don’t understand why it should. 
And I assume although I just did ask him in an email, have you rebooted the 
phone? Other than those two things, is there anything else he can do?
