I think I might know what could possibly be the issue. When speech
volume is set to 0 in the rotor, you wont get speech unless you're using
Alex for some reason. He will speak if the rotor volume is below 5
percent. So here's what to try. Make sure your phone is unlocked and
turn the rotor and flick up once each time you turn it. I'm not sure how
many things you have in your rotor, but 10 times of turning and flicking
up once might get you to the volume. A lot of people have lost speech
and got it back by doing this. Actually I've given this advice to
several people here on this list over the years and they've had success
with it.

Hi Jannet, try to do restart to your iPhone. To me it helps. I have
this issue but it made it for a while but come back for a few times.
I think that you need to report to Apple accessibility for your
problem. Try to deactivate and activate voiceover again and see if it
solves the problem.
