I think I solved my own problem. I used to use an iPhone 5S and I usually kept it in a cell phone holder that was part of a fanny pack with the speaker facing up so that I could hear it. I now have the iPhone 8 and do the same thing. Since this phone has 2 speakers, I decided to put it in the holder with the sensor facing up and I noticed tonight that the notifications were speaking. I’m learning something new every day with this phone! If so, there's a setting which makes the phone respond differently in some situations if you are looking at it than if you aren't. You may have that setting on. In Accessibility Settings, there's an Attention feature that determines whether this is the case.
