As for the Voiceover disappearing issue in the car, I would simply disconnect 
the car’s audio since it sounds like you aren’t really using it. Since I assume 
you aren’t the one driving you might as well take any calls just on your phone.

If you do not plan to use a given app with bluetooth, say no when it asks.
Also, does the car show up in your bluetooth setting?
If so, double tap on the More Info and forget the device"
Or check the Car Play settings and turn it off there.
Then, it won't connect 'when you get in the car.
It's rather annoying.

could use earbuds and have speech, but the minute I unplugged them it was
gone.

So, here's the question:  Why do some aps ask to have bluetooth turned on?
And is it really necessary if I'm not trying to connect to my laptop or some
other device?
