You can email [email protected], but as Richard said, you do need the transaction number from your email receipt. If the app is actually not accessible I would tell them that your are blind and a Voiceover user and that unfortunately you thought the app was accessible, but after doing the in-app purchase you realized it is not and can you please get a refund. If you don't have an email receipt you can log into your Apple account and look up all your transactions there. If you can't figure out how to find that and you know the exact date when you purchased it (should be able to look that up on your credit card statement) you could always try first to email them, tell them your Apple Id email (not your password of course), the exact name of the app and on what date you purchased it.
