You need either a TV or receiver that has HDMI inputs or an optical input. Both are digital so most old "tube style" TV's won't have them and most receivers that are older than 15 or so years won't have them, either. If you have an optical input but not HDMI you can get audio only from the Apple TV, you need HDMI for audio/video.
