Okay, I called Apple accessibility and they said that the Screen Recognition feature isn't showing in Voiceover / Quick Settings because I'm not running iOS16...
They didn't explain why or why not but, all they said was that it wouldn't as well as other features like Screen Recognition won't show in Voiceover / Quick Settings to ve selected or unselected until an individual has updated to iOS16..., it was an update feature that came along with iOS16. The advantage is that it cannot be accidentally turned on. Until Apple fixes the Rotor so it doesn’t randomly change what is focused, accidentally turning on various settings will happen. This one can reak havvoc, so moving it to Quick Settings will lessen the problems it can cause while leaving it available for those places where it may be useful. To open Quick Settings, do a two finger quadruple tap. Find the setting you want to enable and do a one finger double tap.
