I don't know why it disappeared, but you could find it in your Apps Library and then move it back into the dock or onto a home screen. When you get to your last home screen, do a three finger swipe left to get to the Appls Library. You can then use the search field at the top of the screen, or after tapping somewhere in the middle of the screen, do a three finger swipe up to alphabetize the list. Once you find the app you're looking for, use the normal VoiceOver gestures to move the app where you want.
