Open settings, Accessibility, VoiceOver, rotor, Rotor items, once you open the Rotor Items screen, anything that says the word "selected" is on the rotor. Double tap it and it will no longer say Selected, and won't be on the rotor.

If you use the rotor, make sure it is not in the rotor. That feature is to try and help when an app has accessibility issues, which the phone app does not. Also, double check under settings, Apps, Phone, and near the top, probably 3 or 4 things down the screen is Incoming Calls, make sure that says Full Screen.
