Once again, first eliminate the obvcious, clean the screen and if you use a screen protector, potentially remove it. Also, make sure the person didn’t make any changes to gestures and even if accidentally add a new gesture to trigger Voiceover Mute/Unmute.

I have never had VoiceOver start speaking again on its own after doing the 3 finger double tap, but I have notice that sometimes doing a 2 finger double tap, I've let a finger get too close to the screen and accidentally did a 3 finger double tap...
