Hi Ari, I had the same problem with my 4S and it got worse with iOS 7. Not much you can do about it except use a headset whenever possible.
Just for the record, I also find the automatic switching to speakerphone annoying even though with my new 5S the problem has basically gone away completely and the automatic switching is very reliable. However, I still don't understand why Apple thinks a blind user can't just double tap the speakerphone utton if they want the call on speakerphone just as any sighted person has to do. H I just checked, and the route incoming calls option is set to default. I've always had issues with my 4S and the proximity sensor. It's way too sensitive when it doesn't need to be, and not sensitive enough when it needs to be. Basically, I've always had issues with it going on speakerphone and remaining on that setting, even when I cover the proximity sensor with my hand. It's really really frustrating. Anyway, i've never really had an issue with calls not being routed to the headset until now. The heads that will be working fine, and then, when a call comes in, all of a sudden, it gets routed to the speakerphone. Or sometimes to the phone's speaker, which makes things even more difficult, because usually the phone won't go on speaker phone no matter how far away I hold it from my head. I really don't like the proximity sensor, to be honest. Thanks for the help, though. I don't have AppleCare, so I can't replace the phone.
