Thanks, everyone. mark, to clarify, the issue is mostly with outgoing calls--not incoming ones.
Thanks also for the recommendations about the proximity sensor. it does seem, however, that my phone is particularly finicky in this matter. I'm going to show it to an apple rep to see if anything can't be done.
