Jonathan offered to call you and to take your information over the phone. In order to make this as easy as possible, here is a link which takes you directly to the Contact form on Jonathan's website. All you have to enter is your name, your email and the nature of your inquiry. Since Jonathan is already expecting your message, I'm sure all you have to do is say that you are contacting him regarding his book and could he please call you at whatever your number is so you can give him your information. I guess if you write your mailing address in the message that is one piece of information you don't have to give him over the phone.
