Ben, what I do is, as I try to locate the proper HDMI using my TV remote in
one hand, I have the Apple TV remote in the other and I just press up and
down, up and down so that it is speaking.

In a great world, remote control manufacturers would have an audible or
tactile click with each button press. The sighted people I have mentioned
this to have almost all said that would be a help to them, too. As
manufacturers try shaving every nickel off the cost of everything, the
remote devices seem to be made more and more poorly.

As you say, selecting the correct input source can be problematic,
both in terms of the key sequence required for your TV & then knowing
if you've selected the input that you've plugged the Apple TV into.
I know the key sequence for my TV, so what I've done in the past is to
plug my laptop in via HDMI, leave speech reading a document then
experiment with changing the input on the TV until I hear speech
through the speakers. The reason why I suggest this is because the
prompt that tells you how to turn VO on is only spoken once a minute
from memory, so there will be a large amount of time when the Apple TV
won't be generating any sound which obviously makes it a bit harder to
tell if you've selected the correct input.
