I suspect that in the 5.4 timeframe we didn't assemble anything like
a complete lib/Config.pm, so $Config{extensions} may not have existed
or may not have been very accurate. It got a lot better in 5.5.
Dan's patch was probably a little hack to make sure the test ran on
VMS even in the absence of $Config{extensions}. I think the best
approach is to remove the special case and then just make sure that
the test doesn't get skipped. Sorry, but I'm tuitless to help with
that at the moment.
