Case sensitive will almost surely cause trouble.  I do most of my
development and testing with traditional parse style and no DECC$*
feature logicals defined.  Not sure why extended parse did better for
you, though if you do have, for example, DECC$EFS_CASE_PRESERVE
defined then you might also need extended parse style (and vice
versa).

CPANPLUS does attempt to connect to the internet during its tests, so
if there is a proxy or something else in the way, that could be a
problem.
