When they issue an ETRN it will be accompanied with the Domain name, then
sends it to the IP address of the mail server based on the MX record (I
think - Yves, correct me if I'm wrong please).

Exchange 2000 can also pass username/pass for authentication, however
VOPMail does not appear to support SMTP auth on ETRN or Relay situations.
Your only security relies on the fact that 2 servers can't have the same IP
address on the Internet and that your MX record points to the only receiving
server.
