It was caused because several fail-safe devices all failed, in a very
unlikely tragic coincidence. The recombiner failed, a relief valve failed,
and the researchers did not realize there was built up pressure from
stoichiometric gas. They issued a long report, which I have not uploaded.
Here is a short report which describes some puzzling aspects of it. As
puzzling as these are, I do not think there is any doubt it was a
recombination explosion that propelled the cell upward. Unfortunately, it
struck the victim, Andrew Riley, and killed him.

The PdD did not react in this accident. Only the gas in the headspace
reacted, explosively.
