Not an M$sql person myself (MySQL+php here), but once you create or name a 
database, then the next window should give you more options in regards to 
creating sql statements and adding tables with some gui options for your db 
along with being able to upload an sql statement that you write yourself.

I'll say! Reading a little closer, looks like you won't actually see your gui for 
tables until BOTH a db is named along with a user (step 3) and that is done 
through your hosts' control panel.

Once your db is named and you are logged in as a user, you'll probably see the 
name of the db and hopefully that's a link that you can click on that opens 
everything else up.
