Firstly, you're missing the brackets for include()... should look more like
this:
include ($_SERVER['DOCUMENT_ROOT']. '/file.html');
Secondly, this looks fishy to me:
> $sql = @mysql_query("INSERT INTO spw_type (spw_type_id ,
> spw_type_description) VALUES ('', '$_POST[poemtype]')")
> or die ("Query failed");
I think the single quotes around $_POST[poemtype] should actually be inside
the square brackets, and it looks like there's a missing double quote
somewhere as well.
Try adding
echo "<pre>". $sql. "</pre>";
after that line and you'll see what you're actually sending to the database,
rather just getting the error message.
