What page is the form on? What page is the script on? The script I sent you presumes that the form submits to the page it's on. If the information submitted doesn't make it through validation, the user can go through the form again. If the information does pass validation, the user gets redirected to the thank-you page. Are the form and the script on different pages in your case?
