I tend to use forms to get feedback or requests for more information since I can 
prompt the user for information that they might not include in an email message.  How 
detailed you get in the form depends on what information you need.  If you have 
several areas that need the same sort of information, you can use the same form and 
simply fill one of the variables with info from the section you are in when
you ask for that form.

p.s. it also makes it more difficult for those "steal email addresses from a webpage" 
programs to pick up the address.
