I have had the same problem as have some users of one of my sites. The problem
seems to occur when returning to a form page from soewhere else, usually with
the back button. It seems to be connected to the video drivers, since it
occurs with some cards and not with others.
Minimizing the browser and then restoring it to normal screen size seems to
restore the fields.
