yes, tool along is not sufficient if people don't follow the right
process.

in our environment we have the source code control tools in place
for eons, but, as new people join in the group, we still find newbies
making mistake and wipe out someone else's change.  

yes you have the official source code in one place, only one person 
can check it out at one time. but, while person A got the official copy
and making changes to the code, person B is making changes to the file in
his/her local copy, in different portion of the file.  person A tested
the change and submit it back to the source code control.  person B
checks the file out, /copy/ the local version over the official file
-- replacing the new code from person A with the old code in his/her
local copy!  

what person B should have done is edit the official copy and make
changes /only/ to the portions he has done in his local copy. typically
we won't find out the mistake until after a full build (compile) and 
person A is surprised to find his code change has no effect on the 
product.
