Just to put it into context. I am paid extra by DePaul if I teach in summer. The summer I wrote the book I did not teach. It will take about two years of book sales to recover that loss. This is not a complain. I am happy this way.
Moreover it is not my policy to sell other people work unless it is agreed upon in advance. 7 people have helped with reviews of the book but that is another story and their work is acknowledged. Anything you contribute to stays free. This is why web2py is GPL and not BSD. I will setup the wiki later today or tomorrow.
