If you are going to include the contact information in your NPP you must use the address and telephone number for your regional OCR office. and since I don't know where you are located I can't say specifically but the details are on pages 13711-12 of the Federal Register. Note, however, that you are not required to include this information in your NPP, you are only required to give notice that a complaint may be directed to the Secretary of DHHS. [See 45 CFR 160.306 and 164.520(b)(1)(vi) and 164.520(b)(1)(vii)] Some CE's, mine included, are going to provide the contact information on request but are going to include in our NPP only the contact information to direct questions, concerns and complaints to our internal contact person. We are, in fact, going to do our best to encourage clients and members of our workforce to bring such issues to us in the hope that we can resolve them and thereby preclude most complaints from being directed to DHHS.
