We leave in leading and trailing zeroes, and remove the decimal or dot,
right?

That is not correct. There are not "numbers" but "codes" so by removing the 
trailing zero you substantially change the meaning of the code.  You cannot 
do that.  The trailing zero needs to stay in place.  Same issue for leading 
zeroes, they are significant too.

in HIPAA EDI transaction sets need to be sent without the trailing zero,
i.e. 
428.0 becomes 428, not 4280.
