Follow-up Comment #1:
This is not a THoT-specific bug but an engine problem. Apparently the
implementation of [teleport] does not check if a landing hex is passable
before dropping a unit there. As the original submitter says, if you
teleport a bunch of dwarves through the THoT symbols some will end up in cave
walls.
