With that, no template is needed, since you just plop it into a page in the "Form" namespace, and then load it with Special:RunQuery/Form:Your_form_Name There could be multiple triggers for the same bug - I'm not sure. Either way, similar error messages seem to be fairly common with the Semantic Forms 2.3.2 and MediaWiki 1.18 combination I've been testing. Referata uses different versions, and I was not able to trigger the bug on it. I'm not sure how the backtrace info is supposed to be read, but I think it's supposed to be read in reverse order. Beyond that, or how to make use of it, I don't know. The only reason I blame this bug on Semantic Forms is because the errors alleviate when I disable Semantic Forms. However, it could be some strange interaction between particular versions of SF, SMW, and/or MW. That's the limit of what my bug testing skills can tell me since I'm not familiar enough with the code to guide me towards better guesses, or further tests.
