I've used the <ref> tag successfully within pedagogical templates. See for example: http://wikieducator.org/Educators_care/Ownership_of_ideas I did a quick test. It appears that there is a bug in the <quiz> extension which conflicts with the <ref> tags. (When I remove the MCQ using the <quiz> extension the references behave normally. The alternative quiz widget using {{{Quiz_Multiple_Choice|z01|True or False Quiz| Question formats go here}}}} does not appear to conflict with the <ref> tags, so I suggest that you use this syntax for your quizes. Remember to use unique Quiz IDs for subsequent instances of the Quiz widget, eg Z02 , Z03 etc. I think that will resolve the referencing issue.
