Just looked at Foxit Reader 1.3. It opens PDF files, and under Tools
has an option called Typewriter. With Typewriter, one could fill in
existing PDF forms, and then SaveAs. Price is right too. (I use it as
my default PDF viewer - faster than Adobe Reader).
Would this do what you want?
