A few unused index entries in a CHKDSK report is normal. An image restore is probably the quickest way to fix the problem, if it's software related.

Windows has finished checking your disk. For a while it was fine, but now the problem is back. I've read the thread on the system32 folder and checked startup programs and deleted temps and caches. It's happening more often now.
