It is easy: the ISP knows exactly which IPs are dynamically assigned and 
which are statically assigned [and so have someone actually responsible 
for its use] and can easily block port 25 for the  dynamic ones.

An ISP with way too much time on their hands can log dynamic IP port-25 
attempts and then chase down the luser and try to help them get their 
system straightened out, but mostly you just block the port and forget 
about it.
