Interesting that you bring this up. I saw a similar thing a couple of weeks ago and I have not had a chance to track it down.
We are using Aruba , Cisco Secure ACS, WPA/TKIP. I saw this when I was tracking down an issue with an Apple MAC but saw it with other devices as well.
