Another hint as to what might be the problem: when applying the patch as with a non-admin user (Guest) or a user with admin rights, Vista displays the dialog saying an Unknown program needs permission to change my system. I do *not* see the Manufacturer name from the certificate. Does this indicate something has not been configured or signed correctly?
I feel we are so close with Wix and we have seen that InstallShield Pro can do what we want to do so it must be possible. WiX will save us lots of $ and I prefer the WiX way of doing things (especially when it comes to patching and how we can easily automate all of this).
