Paul,
 Grab another NIC for it, so you have 2, then bind one of them exclusively
to your internal IP settings.
 I don't worry about the forwarding bit because my box seems to handle that
itself, I just have TCP ports 20 and 21 forwarded to my internal IP...
 This may sound stupid but is any part of IIS installed? Are you running
NTFS?
 Do other protocols work for you from outside at all?
Have you kept up with service packs and hotfixes and all?
Try reaching your FTP server by both its internal and external IPs and see
if you get the same behavior as an outside user... It could be a permissions
problem if so.

Sometimes WIN2K/IIS/NTFS permissions like to make life a living hell, and
that can include resetting their configs by themselves quite regularly, or
things take 2 or 3 tries before they're all set. They can also interfere
with third party server settings in some cases. You can also go back and
forth between an NT and WSFTP database and try it all again and see what
works.. ( yes, it is a major pain in the ass... )

Make sure the IUSR_ and IWAM_<MACHINE> accounts have access this computer
from the network and access this computer locally user rights in addition to
the folder rights you want to dole out... and, obviously, double check
WSFTP's permissions for your anonymous account to see what it can and can't
do regardless.
