Hi Joe,

the first issue I see is that the platforms/qwindows.dll plugin appears 
to be for Qt 5.5 but the main Qt DLLs are for Qt 5.2.1 -- they need to 
be from the same Qt development package.

Also you need to add a qt.conf file to the same directory as the main 
executable containing something like:

[Paths]
Plugins = .

I am not 100% sure that it will work with no parent plugins directory 
but I think it is ok. If you build another package with the above 
changes I can check it again.
