Bad news.

The last lines of the message log show that it's hhc.exe (part of 
Microsoft's HTML Help Workshop) which fails and reports "runtime error 
R6002 - floating point not loaded".

There is nothing we can do on our side to fix this problem. This problem 
(which we never had, even with very large and very complex DITA 
documents) is almost certainly caused by your working environment.

I would suggest:

[1] To uninstall HTML Help Workshop.

[2] To reboot.

[3] To download latest HTML Help Workshop from

http://www.microsoft.com/download/en/details.aspx?displaylang=en&id=21138

[4] To install it.

[5] To reboot.

[6] To retry the conversion.

[7] If [6] fails, to retry the conversion with your anti-virus software 
temporarily disabled.

OR

To try the same conversion on another machine.
