This used to happen to us quite often. My suspicion was always that the virus scanner scripts we were using were contributing to the problem. We no longer see this occuring since we have replaced those scripts. However, we also upgraded to a newer version of xmail at the same time - so the exact reason we no longer see this is uncertain.
When we did have to deal with this - the fastest solution for us was to stop the server, and wipe out the index files, and the entire spool structure, and allow xmail to recreate them upon restart. This of course only works if the downsides are not overwhelming to your situation.
